ERICEIRA WAVES SURF SCHOOL
ASSUMPTION OF RISK
I understand that surfing involves physical risks, including but not limited to: drowning, injuries from impact with equipment or the sea floor, currents, contact with marine life and sun exposure. Sea conditions can change rapidly.
PHYSICAL CONDITION
I declare that I am fit for water sports activities. I do not suffer from heart conditions, epilepsy or other conditions that may be aggravated by the activity. I commit to informing the instructor of any limitation or medical condition before the lesson.
SAFETY RULES
I commit to following all instructor instructions, staying within the designated water zone and not consuming alcohol or substances that affect my physical capacity in the 12 hours before the activity.
MINOR STUDENTS
If the student is under 18 years of age, the parent or legal guardian assumes full responsibility for the minor during the lesson. The guardian declares that the minor is fit for water sports activities, that the school has been informed of any relevant medical conditions, and authorizes first aid and emergency service contact if necessary. The guardian's signature is required.
LIABILITY
I release ERICEIRA WAVES SURF SCHOOL from liability for accidents resulting from non-compliance with safety rules or omitted health information. I authorize first aid and emergency service contact if necessary.
MINORS
For participants under 18 years of age, the parent or legal guardian declares that: they authorize the minor's participation in the surf lesson; the minor has not consumed alcohol or substances that affect their physical capacity in the 12 hours prior; they commit to informing the instructor of any medical condition or limitation of the minor; in case of emergency, they authorize first aid and contact with emergency services.
IMAGE RIGHTS
I authorize the use of photos and videos captured during the lesson for the school's promotional purposes, unless otherwise communicated to the instructor.
PERSONAL DATA
My data is processed under GDPR, exclusively for booking management and safety. I may exercise my rights by contacting the school.
This document is valid for all lessons booked via SurfBooking.
