I, the undersigned, declare that I voluntarily participate in surf activities organized by Future Eco Surf School. I confirm that:
1. I am in good physical health and fit to practice surfing.
2. I have been informed about the inherent risks of the activity, including drowning, injuries and contact with marine life.
3. I commit to following all safety instructions given by instructors.
4. I authorize the school to provide first aid if necessary.
5. I release Future Eco Surf School and its instructors from liability for accidents resulting from non-compliance with safety rules.
6. I confirm I am over 18 years old or have legal guardian authorization.
This declaration is valid for all lessons booked through the SurfBooking platform.
